The NPI was implemented on December 17, 2007 for all Medi-Cal providers. Effective December 17, 2007, the NPI can be submitted in place of the Medi-Cal provider number.
2. Where would the provider representative signature go on the UB-04 claim form?
The provider representative signature is no longer required on the UB-04 form.
3. Are the attending, operating and other physician’s first and last name required on the UB-04 claim form?
This information is not required on the UB-04 form.
